Transitions

Transitions' mission is to empower young people with intellectual and developmental disabilities through self directed goal setting. Transitions will focus on independent living skills and community engagement activities.

Transitions begins with a professional staff meeting with each individual and their family for data collection. This information share begins the active process of setting goals and identifying areas of interest to the participant. Through a self-direction process, individuals will actively engage in setting their own goals and identifying areas of learning that are important to them.

Services are available for 15-30 year olds. Flexible hours can be arranged. Afterschool and early evening hours will be emphasized for 15-21 year olds. Post graduate participation will be based upon daily work schedules and other activities.
